Fisherdwarf falls into a chasm?

I am still west of the river yet I got a message that a fisherdwarf fell into a chasm... it's now on the deceased list?? gotta be one dumb dwarf eh?  ;)

Hahahahaha. Sometimes there are little patches of chasm near the waterfalls. Sounds like you were just unlucky.   :p

AH! Yah he was near a waterfall hehe. I forgot to lock the lower door when I hit a spur of the river. Thanks for the info it's good to know... statues all around it or something next time.

Wierd, the only reasons I can think of for a dwarf to fall into a chasm (especially near the river!) is...

...s/he was stark raving mad and didn't watch where s/he was going.

...a natural flood pushed him/her over the brink.

Happened to my manager...  swept into a 1x1 mini-chasm near a waterfall by a flood.  You can just bridge over them once you know that they're a problem, though.

...hmm, can dwarves use these tiny holes to chasm items before you reach the chasm?  Can chasm monsters pop out of them?  Having chasm monsters pop out seems a bit harsh...  It probably doesn't connect to the main chasm anyway.

Monsters also push dwarves into chasms.

The option doesnt show up until you hit the main chasm. Also Ive never seen them carry any there. I do have a floodgate/channel for my first farms that passes a chasm square like that and the water doesnt seem to trigger any agrivation.

To add to my forgetfulness: dwarves bumrushing  monsters into the chasm! Or worse, charging at them and having the monster dodge aside.
