Not my own death but...

I dominated a low level orc that I liked the look of, and wiped out half the orc leadership feeding him kills to get him to max rank (20). Finally once he hit max rank I ordered him to become the bodyguard of a warchief.

His mission to prove himself was a beast hunt. I go there to protect him, it's a graug. As soon as I get control of my character I sprint forward to help.... and the graug just reaches down, grabs him, and eats him. 1-hit insta-kill before I could even get in range.

And then of course (spoiler for 6 year old game? ok)

Shadow of mordor. Such a great idea for a game, the nemesis system is such an amazing concept but it just never works right mostly because all the orcs die so damn easy. Oh well, I still had fun with it despite the flaws - it's a great mindless power fantasy if nothing else. I've heard shadow of war is better, I actually own that too but I wanted to replay SoM first because I realized I didn't remember anything about it from my first playthrough except that it was too easy.

Fighting on two fronts like that can certainly end a game.  My answer to that unfortunate situation is to invest in fortifying the starbase(s) along one border (sometimes even writing off systems back to a chokepoint) and lovebombing the other antagonist with envoys.  Even a pocketed militarist empire will be surprisingly cooperative, in my experience - unless they've already rivaled you.  I suggest sending envoys before it comes to that.
Two guys loathed me. I was allies with the wrong people, and they were allied with the wrong ones too.

They'd have never gone for me, I was in a triple defensive pact and had a (weak) vassal. I tried to vassalise one of them, but there was just too much.

Could probably have dealt with it if I'd kept my fleets together, but even though they were nearly adjacent it was a trip through enemy territory which slowed my second fleet down too much, or a far too long trip through friendly territory. The hyperlanes shook out in a bit of a bad way, I'd have had a single front with the guys I was trying to vassalise were it not for the Rubricator system creating a nice hole into my territory for them. Third fleet was occupied with making sure their allies couldn't go through my vassal to get at me.

Somehow built a lead mine next to an ammo belt feeding copper into my gunwall of Salvoes. Salvoes cannot fire lead, so the line jammed up... right in the middle of a wave. Since it's the non-random map Overgrowth, the time between waves is very short, so there's no time to fix the belt and fight off the enemies that get through. Ragequit and lost about an hour of work.

Further on this. Went back to that battle, which loaded the save in middle of the wave I quit in. Quickly dealt with the enemies as none of them had anti-air, and the next wave was just those weak suicide drones. Fixed the ammo belt, expanded my defenses and production. Managed to make some headway, even being able to make bombing runs on the enemy core.

Then I had to deal with cost me the previous attempt on this map. The boss around wave 35, a giant walker. This time, I got it to the tiniest sliver of health because it was in the firing range of two Meltdown lasers, only for it to regenerate from killing my buildings. Put another Meltdown by the core, and would have killed it if my core had held out for a few more seconds.

On the plus side, when I dropped into the map this time, several of the buildings were still intact while there were ghosts for the rest, while the enemy base didn't recover from the first attempt. Third time might well be the charm here.

Opened a big weapon chest. After clearing out the nearby rats, I checked what was in it. Three screwdrivers.
That's not a run killer or anything, but it sure is a 0.005% chance for me to roll the worst weapon three times in a row.

Playing Breath of the Wild. Attempting to sneak up to some monsters. Link, for some irrational reason, decides to stand up and whistle for his horse. Cue five blue bokoblins, two red bokoblins and a blue moblin smashing Link into a pulp before I can process it.

I managed to get myself softlocked. There were two blue one-way gates facing towards each other, with a morph passage between them. I shot them with wave beam, then morphed and went. But I accidentaly shot them twice, and they closed before I was through. So now I was in a morph passage, entirely alive, but to get out I needed to shoot something with the wave beam. Which I could not use until I got out.

Was giving the Covenant a decent beating... for once. Helps that instead of taking Admiral Hood as my fleet admiral, I took the ONI guy. This eventually gives access to the Thanatos-class battleship. This thing is awesome. Two of these knocked out a few heavier Covenant ships with minimal damage. Even my early expansion fleet, consisting of two Marathons, an Orion carrier, 9 Paris-class frigates, 5 Charon-class, and a repair ship, took out an enemy capital ship losing mainly just the repair ship.

Then for some reason, both my keyboard and mouse disconnected and then reconnected. Might have been all my USB ports doing that. This happened before when I was playing the other day. I alt-tabbed out to see if this happens to anyone else. And then I discovered alt-tabbing locks up the game.

Sounds like a great mod!  I haven't played SoaSE in a long time, shame about the alt-tabbing though.

Kenshi is a wonderful game, I just wish they'd used a prebuilt engine rather than making their own so I didn't have to deal with loading times and pathfinding that, say, makes my guys break down a shop door then stand in the doorway when I told them to go to another city.

The Gaping Dragon isn't even that hard, it's just that I got greedy, got clipped by the beginning of his charge, and then got stuck in a flinch animation while a much scarier hitbox plowed over me and deleted the rest of my HP bar.

There’s a charm to the first game that isn’t replicated in any of the sequels. I hope it clicks for you eventually. Are you playing original or remastered?

The original. From what I've seen the remaster doesn't change a whole lot.
I will say one thing for the game; as often as it finds some creative way to hurl me off a ledge to my doom, it has really, really good level design. The way everything links together is downright genius.

My most recent character is kinda stat-spread all over the place because I wasn't sure what I wanted to do, so I'll probably make a new character soon and focus on Dex and faster weapons (and maybe pick up pyromancy for fun and profit). I've played through the first few bosses of the game enough times that I typically wipe the floor with them provided I don't make a dumb mistake.

A question, before I do make a new character and forsake all bonds of fellowship with Str-based stuff: is there actually a good reason to use the big, clunky weapons? From what I've seen of them thus far (zweihander, great club, and so on), they're all downsides--your attacks are extremely slow and punishable, you have to put points into Endurance in addition to Strength just so you can swing more than twice, and you can seemingly deal just as much damage by enchanting a dagger or shortsword and just going to town with it. Why would anyone use an ultra greatsword, even just in PvE?

Well first of all, you're going to want to put points into endurance anyway. It's considered the king of stats. It increases equip-load which isn't just useful for heavier weapons, but it's also useful for wearing stronger Armour (therefore you effectively get an hp increase). Plus, you're going to want to increase stamina at some point.

As for using heavy weapons, you are missing two key details. The first is poise damage. You know how poison builds up when those rats attack you, but you only get poisoned when the bar is full? Well the same thing applies to hitstun. Every weapon in the game does a certain amount of poise damage which slowly fills up an invisible bar every time an enemy is hit by it. Once the bar is full, the enemy stops whatever it is doing (including attacking) and is momentarily staggered as the bar resets. Heavier weapons deal more poise damage and lighter weapons deal less. Smaller enemies are staggered easily, while bigger enemies like black knights and bosses usually require a few hits from big weapons to get interrupted. To compensate, bigger foes are usually stunned for longer once their defenses finally do crumble allowing you to momentarily wail on them without fear of retaliation. There really is nothing more satisfying than charging at a boss who's winding up a big attack, while knowing that it won't hurt you because your next hit is the last one needed to break its poise.

The second thing you're missing is the fact that UGS's are usually late game weapons. You get a few big weapons early on like the GreatAxe and the Greatclub, but those are shit and they have terrible movesets (I've never used the Zwei, but I know the community loves it for some reason). I was mostly a halberd dude until I found the Black Knight Sword (not the GreatSword). That thing was a beast that owned even without any titanite. So keep going, you may find something you like eventually.

Finally, I want to mention the fact that Strength weapons are meant to be used two-handed. In addition to improving the move-set, wielding a weapon with two-hands increases your effective strength by 50% which grants them a decent damage boost compared to their one-handed dexterity-based counterparts. I noticed you complain about strength weapons doing underwhelming damage which to me is a sign that something went wrong. The whole trade-off with them is that you deal more damage (both per hit and per second) at the cost of being less flexible than you would be with dexterity weapons. It's easy to underestimate attribute scaling, since you only see it when you wield the weapon, but weapons with larger base damage benefit from scaling more. They also get a higher damage bonus from titanite.

you can seemingly deal just as much damage by enchanting a dagger or shortsword and just going to town with it. Why would anyone use an ultra greatsword, even just in PvE?

If you meant enchant as in temporarily buffing your weapon with spells like "magic weapon", "lightning blade" or "greater magic weapon", then you do have a point there. The damage increase from those spells scales solely with your Spell Power (a function of your wand and your INT) without taking weapon base damage into account, making it optimal to use those spells on fast weapons. If you're casting those spells though, then you're playing a wizard and INT is your primary attacking attribute. You have no reason to level STR in that case, and thus no reason to use bigger weapons.

Finally, let me finish with a basic guide to DS1 levelling.

1. Never level up Resistance, (trust me on this one. It's a terribly designed stat)

2. Leveling is usually about choosing between HP, Endurance, and your primary attribute. If you use weapons that scale best with strength, then your primary attribute is STR. If dexterity, then it's DEX. If you're a wizard, then it's INT. If you cast miracles, then it's Faith. You can rarely go wrong if you consistently make a choice from one of those 3.

I hope you have fun.

-snip-
Wait, you're telling me bosses can be staggered? Holy crap, sign me up. I never saw one stagger during all the time I played, so I figured I'd be swinging a giant sword at an unfeeling HP bar with no remorse and no regard for my laggy attacks. Now that I know I can smack 'em around with the right equipment, I am 100% on board.
