I didn't play Multiplayer and I didn't do out of game stuff (which wasn't advertised ingame anyway), so my overall war readiness stayed at 50%. I thought it might have something to do with the difficulty, and it wasn't explained ingame, so I ignored it.
I died in the ending cutscene.
Who thought this was a good idea?

Mass Effect 3 is all about pyrrhic victories and a growing understanding that your fight is probably hopeless. It took everything you had to stop one Reaper in the first game, and the galaxy still hasn't fully recovered. Taking on the Collectors--not even a Reaper, just their flunky invasion force and a half-built prototype--was considered a suicide mission at the end of ME2. You're preparing for extinction as much as for the final battle. Shepard suffers frequent nightmares throughout ME3, possibly an attempt at portraying PTSD in an action game. The costs of victory, either potential or unavoidable, grow higher and higher as the game goes on until you're nearly burnt to a crisp and practically dragging yourself across the battlefield--potentially after watching two more party members die, on top of everybody else. It's not the game's fault that you expected an unambiguous, totally happy ending.

But IIRC you just need to get 100% single-player completion, or very close to it, and it's revealed you survived in a post-credits stinger. I tried once but it's a bastard with all the scanning sidequests.

Didn't they change the ending to something happy because people were so upset about it?

I played the game many years after release, so I only know the "updated" version. Even back then I knew better than to take gamer "outrage" seriously, so I didn't read any articles at the time, either.

I'd be perfectly fine with the original ending and Shepard dying. I am not fine with gating a better ending behind stuff that can't be done within the campaign.

What decides if the squadmates die before ascending to the citadel? Mine survived (Liara and Garrus).

Basically low completion rating/war score/whatever it's called. I've never seen it, but I imagine that dodging sidequests and making poor storyline decisions will get you there. Who appears in that particular scene just depends on who is available, I think.

Besides, as outlined in this QA thread multiplayer is no longer required. It's extremely difficult and requires a specific choice on the Citadel at the end, but it's possible without multi.

I made the Illusive Man kill himself, didn't help me. There is a kind on minigame where you send "fleets" to complete things (just wait 1 to 5 hours, depending on the mission, rinse and repeat), which then increase the readiness. I might replay a bit to see another ending.

For the curious, I poked around a bit and found a fairly concise outline of how each ending was changed: https://gamerant.com/mass-effect-3-new-ending-changes/

...Not having actually played any of the MEs but just reading some of the issues, I kind of agree... some of the things sound like they replaced the guys in charge of lore and they didn't read the notes.

Well considering Drew Karpyshyn was Lead Writer for Mass Effect 1 & 2 and then left Bioware, I guess that it sort of what happened

Stlill enjoying Circadian Dice with gaining lots of stars from all my characters (i unlocked them all, happy me) i unlocked most scenarios by now, for fun decided to give a try to one of the last , despite it should be over my level (as there are several scenarios i have not yet completed) .

This one originality is that it features an overpowered "cosmic horror" that is frozen for a long amount of turns, so the goal seems to beat the whole scenario before those turns run out and the thing can actually attack (as it can hit for 10 points and have an absurd high health of 20, it's not something easy to deal with). Reminds me of "Dark Depths" card in old MTG.

Wouldn't be that big of a problem (relatively speaking as all enemies aren't weak in this mission) if that scenario didn't had a lot of those guys :

that debuff power will actually lower the amount of turns by 1 each timeon the cosmic horror when it will deal damage to you.

So when you reach a point in which those guys are numerous and you have no dice with any shielding on them :
Spoiler (click to show/hide)

while you're low on health and still not leveled enough to have 4 dices, that's hilariously impossible :D

I'll have to try to increase some more characters stars count in lower levels in order to unlock more items that will very likely help.

Anyways, what a really good game this Circadian Dice is , it's been a long time i haven't got addicted to something like that, can't believe i only ran into its itch.io page by random luck as i never heard about it anywhere.

I'm guessing Sparks died. I say "guessing" because when I loaded my save file, he simply wasn't there and I have no idea where he might be. I remember him being with the party when we left Destard, and I don't remember explicitly kicking him out.

I'm not upset about it--he was only really good for holding torches, and with him gone I could pick up Shamino--but very confused.

A few deaths:

Accidentally backed out of a high window and fell to my death during Live With Killbane. In my (flimsy) defense, I was distracted by one of the Decker Specialists while she was sliding on her knees and leaning back. Truly, one of her deadliest moves that doesn't involve a hammer.

Had to retry several times the hard version of the Snatch activity in the NW sector of the city, due to the escortees' non-understanding of "get in the goddamned car". Said activity automatically maxes out your aggression level with the Deckers the moment you attack them or approach any of the targets; this results in the disconcertingly-fast arrival of multiple SUV-loads of tween edgelords dual-wielding SMGs from all directions. Sometimes, they'll have Specialists or Brutes(oftentimes with miniguns or flamers) hitched to the back. Consequently, I'd prefer to get the hell out of there quickly.

Not me except dying a little inside, doing the final mission, Three Way, fighting my way up the statue. Going up the statue, there's three random Saint mooks you can untie and have backing you up. I get up to the top level of the scaffolding and yeet the last of the bombs off the statue, but a minor error in the mission script made me run back to a previous waypoint(the game didn't register me as passing the waypoint because I passed around it).

After touching the waypoint, the game realized I had finished clearing the bombs and directed me to the final portion of the mission. As soon as I turned around, I saw about about 10 STAG troopers standing over the dead bodies of the three Saints who were backing me up. When the game triggers the objective after clearing the bombs, it also dismisses your followers, meaning they instantly lose all homie bonuses, such as improved health and more importantly the ability to be revived. Just one last kick in the balls by STAG before I kill off their second-in-command, I suppose. Those guys needed a hero, too.

Caltrops are the nastiest trap I've seen in a game. Don't show up to Detect Trap, don't respond to Destroy Trap, practically invisible, and I'm not clear what defends against them because it doesn't seem to be the quality of shoes. Nothing like having to stop a dungeon crawl because Iolo got a booboo on his foot.

You can, but so far I haven't managed to spot them without stepping on them a whole bunch first. And U7's friendly AI means that if one person steps on them, everybody does.
