Many, many times. But the latest one stands out in particular.

After several hours of attempts, I finally got a good run going. I'd never made it past the jungle before, but this time, I started in the mines and made it all the way to the temple. A handy shotgun (thanks Ash!) saved me many times, as did my climbing gloves and cape, and several risky damsel saves had brought me from a terrifying 1 HP up to a far more reasonable 3 by the end of the ice caves.

Then, I got to the Temple. 4-1 spawned as a dark level... with Anubis directly to my left, and a lava pit to my right.

I proceeded to murder Anubis with my trusty shotgun, and then decided to try out this scepter thingy. Spamming its attacks unlocked several new journal entries, as various as-yet-unseen denizens of the temple died instantly to my off-screen homing blasts. I was a god, on top of the world; I'd conquered the worst this game's RNG could throw at me, and I was certain I was about to reach Olmec in my first day.

Apparently, the game noticed my hubris. One of my homing blasts did... something involving levitating a caveman. Next thing I know, I've been killed by my own psychic blast.  :'(

Spiked door trap instakilled me in Barrow Den Falls Bleak Death Place (I honestly can't remember the name).

Skyrim. It was space year 2011 when it came out, why are the graphics options in a separate launcher app instead of the in-game options menu!? >:(

Bleak falls barrow is the place, I think.

Probably because changing the graphics could require the entire game to reset/reload/whatever, and they were feeling lazy they decided that the best way to deal with that probably would be to allow you to change the graphics in a seperate app instead of in-game.

That'd be my guess at least.

There was a time period where that was the industry standard.  Not sure if it still is, haven't bought AAA games recently (lord knows my computer doesn't have the memory left for that).

I find a stele that gives the bonus "monsters don't notice heroes". I'm like, that's awesome! My guys can kill them and they won't even notice they're dying!

Turns out it just made the monsters walk past all my guys and destroy my crystal while my heroes desperately tried to kill them and were ignored.

Could be worse.  Once I had a checkpoint right after Kat drove off a cliff in Reach.  After several minutes it actually set me back to my last checkpoint, which was pretty neat. 
It wasn't even one of those checkpoint traps where you can make an effort of surviving.  She skidded off the cliff back side first.  All jumping out would do is end up with me dying away from the warthog.

For some reason, frost spiders in the wild are much much stronger than ones in dungeons. Frost spiders in the wild are as tough as some boss characters, while the actual boss version of the frostbite spider is nowhere near as tough.

Skyrim. At first I really was not impressed, but now I'm convinced that I need to buy this as soon as the free weekend is over.
