Fell asleep with my hand on the 5 numpad key while there was a dog nearby.  :(
I was doing so well too... I had so many skills trained and an abundance of supplies.

A couple of my friends and I started another Dynamic Universal War System game near one of the massive green towers you find around Altis. After realizing that setting up weapons on these things just drops them to ground level inexplicably, we were equipping to perform a "search a crash site" mission in an enemy zone. Then the raid sirens blared and we got a message that enemies were approaching the base.

Cue an OPFOR APC armed with a grenade launcher storming into our base and opening fire. Just before I fired my Titan launcher, it blew me away, flattened the base, and killed the commander. All of ten minutes and we were screwed to high hell. Campaign failed.

I was ambushed. It waited in the vent until I started to move from my hiding spot, then it jumped down and charged before I could react. Now I have to add "Can plan an intelligent ambush" to the 'Things That The Damned Thing Can Do' list

No matter what I do, the game crashes. I fight the battle manually, the game crashes at the end of next turn's reports(which are almost always the same, even the random stuff), I use autobattle, the game crashes immediately. I'd retreat into the fort, but that might crash the game and would otherwise pretty much guarantee a loss of a major staging area for my war against the Golden Horde.

The most recent save before that is over an hour of progress lost.

Not exactly how I "died", but how I rendered the round unwinnable before commuting suicide.

I was playing Jester, which means my goal was to get the Town to hang me. Day 2, I pose as a Sheriff (Which determines whether or not a person is mafia), and declare a random person to be maf. He gets hung. Turns out he was the Godfather, which is one of the most dangerous and elusive mafia members (They actually come up as innocent, but nobody realized that). Day 3, declare another person as mafia. Turns out he was the mafioso-turned-godfather. At this point, they believe wholeheartedly that I'm Sheriff, and wouldn't dare hang me.
