So I'm breezing through the dungeon, cutting ice and shadow dragons down before my boyz can even reach them, when I come across a room full of slime creatures. They immediately start merging together and form a titanic slime creature within a few turns. I was at 89/121 hp and had 30 AC, so i thought i could chop at it once before backing off.

It killed me in one hit.
The damn thing was still only labeled as "dangerous" and it did 92 damage in one hit through 30 AC. Apparently merging doesn't cause their HD to go up, solely for the purpose of tricking people who've never fought one head-on before.

Got the Enterprise shot out from under me by another Constitution class ship and a bunch of pirates.  On the bright side, no fifth movie.  On the down side, no second or sixth movies.

Half-Life 2 is an amazing game, but the entirety of Anticitizen One would be so much more fun if they didn't force you to work with braindead AI "teammates". I charged down a corridor into a squad of Combine. The problem came when my AI "teammates" blocked my escape, so I was caught between my stupid "teammates" and the Combine that were rapidly eating my health away. I'd ordered them to stay around the corner (and thus, out of my way) less than five seconds prior.

These idiots have the memory of goldfish. For all the praise they heap upon Gordon Freeman, they sure don't listen to him very well. If you order them to stay somewhere, they'll get lonely and come running back after you before ten seconds have passed. They're not terribly good shots either. But they're just durable enough that they're unlikely to be killed, and there's always a medic in every group of "teammates" you encounter. From what I've seen, they don't even trigger hoppers. I always love finding a machine gun turret, because that means being free of "teammates" for the next encounter or two.

Half-Life 2.

There's a lot of triggers that cause them to revert to "follow", especially when you enter new areas.

The weird thing is, the Rebels (And the Combine) actually have very GOOD AI, but none of the levels take advantage of it. The Teammate AI is outright terrible, though.

The part in the car level where you stop at a lighthouse and have to fight off a bunch of Combine that keep dropping in was amazing. That blew my mind even more than the bridge part. It's only when your "friends" start following you down cramped corridors that it becomes a problem...

I ended up with the Skaarj final boss. The guy gains adrenaline so fast, and manages to find armor so very quickly, that any time I end up finding him on the map I end up getting massacred.

What weirds me out about HL2 is that right when you get followers they start adding areas that said followers can't deal with.  If Anticitizen One is the level I'm thinking of, the first thing you have to do is jump down a pit into a cluster of mines, mines that your AI allies are too stupid to avoid.  Then there are the extremely tight corridors that the AI blocks you in, the laser artillery thing they don't try to avoid, the segments of respawning enemies you have to run past that the AI loves to stay and fight, the snipers, the machine guns, the autoturrets, the gunships, the striders...

Really all they had to do was be uncreative and say "there are combine soldiers here, go kill them."

Should've just stopped playing for a while after escaping the Abyss, it made me reckless and stupid.

I've found the opposite when I play RLs.  If I stop playing and then come back, I'll make some stupid mistake and die.

You misspelled seance.  Are possessing Draignean?  Are you actually a ghost in the shell? You have to tell us if you are, that's the rule

Ugh, the laser artillery... I spent far, far more hours on that than I should have.

I've seen videos like this one demonstrating the A.I's ability to cover each other and flank enemies.

It is kind of odd they didn't make many areas that allowed the A.I to stretch itself about. Though, I will say a possible problem is the player headshotting them all before they can do much more then move forwards.

I got locked in a duel to the death with an Enderman. I beat him down to 8/40 HP (with help) but then I started lagging causing me to flail around helplessly as he destroyed my soul.

not sure if my ssp.applies to my limited multiplayer time, but I switch render distances when lagging too long or hard.  Right in combat.  Stupid chunks wont get me!
