Golden Sun: The Lost Age, at what I'm pretty sure is the final boss. He's certainly a massive dick like a final boss should be; he went One Winged Angel on me, and flashed the Djinn Fuckery I forgot what it's really called spell that drains all the stat and power-boosting spirits associated with all your characters (which among other things cuts their HP and defense way down,) and then Cruel Rain is the "I'm a Boss Fight Fuck Everything" doom spell.

In the same turn.

I didn't immediately die since I was able to swap out injured and downed characters for the B-listers, but eventually I lost everyone capable of reviving my party members. Time to blow my big pile of cash on some Water of Life, I suppose...

Golden Sun: The Lost Age, of course. Preparation and foresight made the fight last a good half an hour longer, but I don't know as it actually got me closer to victory. That Djinn Storm ability just WRECKS everything; limited success with cycling fresh party members in, but they're shittier than the main party and I can only rotate one at a time, which tends to get the Djinn-depleted party murdered in the meantime. And then by the time everyone has their Djinn back up, I only get of one attack or two before he does it again.

Magicmaker. Most recent version(?) adds bees to the forest level. On Master level, they do 80 damage(you have 100HP default) on touch, and it's all too easy to hit a beehive offscreen.

None of the other levels have anything this stupidly hard, although the castle is pretty deadly on Doctorate.

Why does the Bio-Rifle send me flying across the room, slamming me into a corner while the rest of the enemy team tears me apart with miniguns?

Rome: Total Mishap

Not exactly deaths, but I just failed four times in a row at carrying hot springwater to Castle Town. Every time, I was less than halfway there. Forget the Gold Skultulas, forget the Bomber's Notebook, forget the bunnies in Spirit Tracks, THIS is the most grueling sidequest I've ever seen.

Legend of Zelda: Twilight Princess. At this point, I'm wondering if the magic armor is just a red herring and I won't actually get it at the end of quest chain...

Had my HP slowly ground out of me over a few hours by nightly spider attacks, until I was finally killed.
Then I respawned, jumped down a worm hole to a different section of the map, and got instantly KO'd by a tentacle.

Haven't played in a few months, still getting used to the new mechanics.

I hit the wrong hotkey and brought my squad up to standing position, this enabled a pair of .50 cal wielding super mutants to turn them into steaming meat chunks
The only survivors were my medic and sniper, and while attempting to link up with the medic, my sniper ran afoul of a mine

I think I'll see how far I can get with just my medic before I restart this mission, maybe I can kill them all with kindness and drug overdoses

I did it only once, and I'm not sure if it wasn't really a hallucination, but I think I remember catching some kind of rare fish that the fishing guy paid me 50 rupees but didn't put it in the tank where your record catch was because it was so rare.
