Drone jam caught me between my five companions and the machine gun turrets I was carefully trying to maneuver around. If only NPCs were more receptive to the idea of listening to commands and getting out of the damn way.

Half-Life 2.

Just speeding around Algonquin on a scooter, hit a cop. I jump off and start kicking him to death then get back on and speed off. Start shooting any cop I see and soon I'm at 4-star wanted level, soon I get stuck between a wall and a car. I jump off my scooter, shoot the driver of the sandwich car and sprint off through an alley, for no reason start slowing down then I get hit by a taxi, around 6 cops come chasing after me and start shooting. The taxi driver was clearly angry so he proceeded to speed up and send me flying into a horde of police, I end up getting shot to death then shot some more.

I would never get arrested, I can't afford to buy all my guns and ammo again.

I got to the very top of the dark energy reactor, and assumed that I was supposed to shoot down the gunships and my objective would be further elaborated on from there. When it wasn't, I was left to shoot cores into and around the glowing energy effect and generally be a clueless dummy until I failed the timed objective. Not a proud moment for me when I have to resort to a walkthrough to complete what turned out to be a very simple segment in the last few minutes of the game.

Half-Life 2. I beat it in about 3 days, and it almost feels like a shame to go back to playing other games since this one blew most of my game collection out of the water. Need to find a way to play the episodes ASAP.

Expeditions: Conquistador. It may not be a TPK, but it still messed me up pretty bad. Especially when the other doctor died a few days later from getting cut up with a macana.

I read that as the other doctor dying of a cut from a maraca.

Okay. I found that Psi reaver you talked about Jacob. Also, remember how I was hurting for AP rounds? I have 90 some of them. In like the final level with pretty much zero chance of finding bots. FUUUUUUU!

Depending on where you are exactly in that level, I'm pretty sure there's some midwives hanging around. AP rounds work on them.

I walked in on Super Wrath on the first floor of the game. He threw a homing troll bomb that I couldn't escape from. It blew up my one heart and killed me.

I hadn't even gotten to the first treasure room yet, jeez. Uncool, game, uncool.

Metal Gear Solid 2 is a lot harder than I remember it being. I was trying to hide underneath a security camera overlooking a door, but apparently I was close enough to the door to open it. A guard happened to be right on the other side and saw me. I was playing on Hard with "Game over if discovered" on.

Metal Gear Solid 2: Substance. From my old save files, I've apparently beaten the game on Extreme and 100%'d several characters in the VR missions, so I guess I just have to get good at it again.

I used to be extremely good at MGS2, mostly because it was the only Xbox game my brother had when I lived with him. I could get very close do doing no-kill, no-alert runs, and was well on my way to 100%-ing the VR missions. Damn those grenade missions, if I could just beat Plisskin's 5th grenade mission I could move on to the next few characters.
