There were so many shiny things on the screen that my computer had to stop and think about it for a minute, and while it did that, I was eaten by a wolf.

There is also an Alter Ego - Male. They are both enjoyable games made by a psychologist who worked with Activision back in the 1980s.

There is also an online version that was made back around 2008 as well. It's the same game but with a few changes to make it more immersive. It's free to play and supported by donations.

My character took two critical hits in a row from a pig rat in Vault 13, right after she had completed that particular area's objective. Now I have to start the damn area all over again because I forgot to save.

Despite using my starfighters' powers of transformation to become a giant robot (And therefore becoming much more agile, at the cost of speed), the Black Fleet Carrier's Anti-Fighter beam cannons easily tracked me down and melted my ship.

Well, at least it is now safe for me to say that it isn't wise to (accidentally) activate a hostile turret and then hold the Up arrow thinking the turret will cover me, rather than spraying me with bullets.

Yeah, I should really probably remove the ability for turrets to be hostile when the player places them.

15 vs. 1 match in Bridge Commander's "Simulation" mode. I was on the 15 side. The problem was, it was 15 shuttlecraft versus 1 space station.

Well, The Boat dragged me under.  I fell to my death and (presumably drowned)  WTF, Guy, you canno swim?  Cost me my feelgoods for the game.

Not strictly a death, but a plan did catastrophically fail.
I trained up 160 Scout Marines, armed 80 of them with Plasma weapons and cloaked them all, planning to walk them all into the enemy base then at once attack.

Got 20 steps away from my Stronghold and the Ork Warboss turned up, instantly decloaking my entire Zerg Marine squad and getting them torn apart by a Squiggoth.

Got ganged up on by at least five Cazadores. Took one of 'em out with a service rifle, though. Then the rest stung me to death.
