And also... REEEEEBEEEEEL FLAAAAAAGSHIIIIIIP!!! Second stage with all those damn drones just murders me. Last game I had a Burst Laser Mk 3, a Ion Mark 2, and a Fire Bomb. The Ion weapon recharged in 4 seconds, minus fifteen percent for my reloader, so pretty much every couple seconds. I STILL got murderated, because I ran out of firebombs.

Indeed. It's so fuuuuun. But makes me so frakking annnnngry. I was in the basic Kestrel, kicking ass and taking names. Bastard rebel flagship, mother... *Mumbled cursing*

I was fighting a half dead night creature, and I was in a martial trance, dodging his attacks and shattering bone with every hit.
All of a sudden, the bastard got in a very lucky hit, and managed to vaporise my head. I got so mad, I deleted the world.

Dwarf Fortress

Shot down a small scout over Scandanavia. Gear up my demolitions expert Captain to take 5 new rookies on thier first mission, which should be easy - after all, its a small scout. A stealthy approach is made to the UFO, and the Captain readies her HE to open a hole in the roof ready for the rookies to rain in the grenades. Alas, just before she can throw it, she ends up mind controlled, and one of the rookes takes her out with a burst of automatic laser pistol fire. Predicably, the HE goes off, wiping out the entire squad.

Was blasting out some tunnels between my base and the Evac shelter, and rigging up sconces every 10 tiles or so for light. I went back to grab some more dynamite and found that the sconce at the start of the tunnel had set the rest of the tunnel on fire, I fled in the opposite direction and found that the fourth one had also set the tunnel on fire.
Cataclysm

Waltzed straight into a dwarven mine field without even realizing it, triggered at least 3 mines before I realized that it was not simply a huge coincidence and that my bacon was about to be fried.

I was just retracing my steps, and almost out the door when all of a sudden, multiple greater dark frogs come in from that exit, forcing me to run wildly out the other exit.
Somehow I survive the rest of the room, but I am now afflicted with the 'scared' status and don't have any scrolls of teleportation left. Another frog shows up in front of me.

I get trapped, then killed in a one-tile wide corridor by frogs, after "You are too scared to attack greater dark frog" x 86.

Playing Dungeons and Dragons 3.5 edition. Eberron setting. We have managed to purchase one airship, and steal two others. We end up fighting a small sky fleet. Our only cleric is dying on boat #1. I'm flying boat #2. Boat #3 is beyond the range of helping out, so it is entirely up to me to get the boats hooked up. DM asks me for a couple of different skill checks to keep the ship under control, maneuvered, and to pull up alongside the other one. Two natural 1's and another roll that is a definitive failure. DM rules that I fly my ship through the other one and end up wrecking them both in a fiery sky death. Farewell, Timothy the half-elven monk. Improved evasion does very little for extreme gravity and ship crashes.

Getting swarmed by zombies, mutants, and a couple robot soldiers, was kicking ass with my drum-magazine rifle when I ran out of ammo. Backed into a corner of weird timey-wimey goo, fired every last nail, grenade, shotgun shell, and bundle of high explosives I had before taking my chances and running into the goo. No remains were found.
