Impaled by the stinger of a giant scorpion. Brand new adventurer, first encounter, hit once with my axe (bruising the chitin) before being killed by the first counterattack

Dwarf Fortress Adventurer Mode, in case it wasn't obvious.

Blown up by 1,000 Fireballs of 10d6 damage each. Rolled 10 saves, for 100 Fireballs at a time, and failed 3. That's 300 item saves per object I was carrying, 1 in 20 chance at absolute best of destruction, which means everything vaporized 15 times.

D&D, playing a rather high level game. I was the only one who died in the trap because I foolishly neglected to account for fire immunity like everyone else :/

Feral charged a mob standing next to a cliff, it was facing towards the cliff, and feral charge places me behind the creature automatically so i wasn't worried.

Unfortunately somebody had managed to pull threat on it right before I hit the button, turning it so it's back was facing the sheer drop.  I landed behind it, with just enough ground for the game to have registered it as safe (if it's unsafe it often just places you on top of the mob instead of slightly behind it) I slid down the sloped ground and off the cliff. 

I tried to shift into my bird form but misclicked in my haste, instead shifting out of cat form, causing me to lose my feline grace ability and causing me to splatter bloody chunks of cow all over the cliffside from fall damage.
