Not sure if it counts as dying, since I landed safely, but last night, was flying in FSX with a Catalina PBY-5A. Yeah... you should really read the manual for those things. The engines are crazy finicky.
To start with, they don't like taking off except from near sea level. Which makes sense, but is still somewhat of a limitation. What looks like an actual engine problem just turned out to be the auto-mixer, screwing up the fuel-air mix for a runway at about 4000 feet ASL. Once I got into the air, things seemed to be fine, for about 45 seconds. Apparently you're not supposed to leave it at full throttle once you're off the ground. More than about 2 minutes of running it at that long and the engines tend to burst into flames. About 60% throttle is all you can really manage without severe overheating, at least until you get higher. My luck, these things glide beautifully and are made for landing on unpaved airstrips, so I found a nice clear dirt road in the middle of some farm land and made a somewhat nice landing there. It was a beautiful sight, me, my PBY, a field of wheat, and two flaming engines.
So yeah, big step up from Cessnas which make it almost impossible to do anything that would hurt the engine.
