Space Station 13.

Accidentally cracked a treasure chest over the elf in our party's head, killing her.  There's no chat so there's no way to really apologize, other than staring at her awkwardly until she respawns.

We make it all the way to the final battle, fighting the last boss.  I'm standing in the front trying to hit the lich with my sword while the cleric spams turn undead the dwarf runs artound being ineffectual and the elf throws knives from behind me.

Suddenly someone tosses a flaming oil at my back, it hits me killing me.  I don't see who did it.

Revenge is clearly a dish best served cold...and then lit on fire.

Go down stairs.  Demon to my left, demon to my right, no way past either of them.  I fire my shotgun, demon still alive.  Fire my chaingun, it dies but another is right behind it.  I then fire my chaingun blindly as both kill me.

In the campaign version of Hoth, you have a time limit of around 20 minutes to complete it. Since I can usually pare my mission time down to less than 8 minutes with some luck and planning, I thought that I'd be able to beat this mission easy-peasy.

I forgot just how brutal the fighting on this map can be. When all was said and done, I wasted about three minutes capturing each of the first few command posts with two minutes of total travel time.

When the time came to destroy the shield generator, I found that my dimwit AI teammates had somehow turned the AT-ATs a complete 90 degrees and were standing completely still. I had to respawn, turn the stupid things around and walk them all the way to the shield generator. By the time I parked the AT-AT just outside Echo Base, I was the only guy left.

As an engineer, one of the worst classes for capturing and defending CPs, I had to take Echo Base from about twelve guys by myself and hold it until some more people spawned. And even after all that I wasn't done. As Darth Vader it was much easier to complete the next two objectives, but I was down to less than a minute.

I was literally right next to the final objective when the time ran out.

Star Wars: Battlefront II

I was happily sitting in lane, soaking experience while under the cover of invisibility. Except I forgot that my Shadow Walk was still at level 1, and didn't remember to recast it the moment its duration ran out.

Cue me suddenly popping into view. While right next to Lion and Ancient Apparition. Who both chained their stuns properly. I almost managed to get into invis before dying, but the last autoattack hit before my 1 second fade time completed.

Stupidest First Blood I ever gave away.

Dota 2.

Playing F.E.A.R, second mission.  I've died three times before on this section, so I sort of know where the enemies are.  First, I get to the point overlooking three of the replica (I think that's what they're called) soldiers.  I throw a grenade, and they scatter.  One of them escapes the blast, and I gun him down in slow mo.  Then I go down and scavenge their ammo.  Next I go the long way around to get the drop on the replicas waiting outside.  I throw another grenade, catching two, and enter slow mo to kill a third.  I then take cover from their barrage of gunfire, as I only have 25 health.  I take out my duel pistols and back up a bit, killing an enemy who comes around the corner.  There are a few left, who I all kill as I slowly advance, making ample use of my slow motion skills.  Victorious, I wait a bit to make sure there are no more enemies.  No one tries to shoot me, so I get out of cover.  Then, the exact moment I do, some guy who I missed shoots me from behind.  The moment I hear gunshots, I hit T, but with no immidiate cover and with needing to turn around 180 degrees to get a shot off, I had no chance.

Hounds attacked and tore through my log armor and killed me. Then I respawned at a touchstone that was half the map away with no supplies and cleanshaven...in the middle of winter.

I froze to death on the way back to my camp. I died twice in as short as 3 minutes of time.

Day 93 as Wilson

I saw saw a Jabberwock(from what i have learned they are very scary, at least from my other encounters with them) and promptly ran, but it caught up with me. By the time i realized i had one bolt in my crossbow it was about 10 tiles away, so i quickly drew my crossbow and shot it. Critical! But it wasn't dead, so i took out my nail board and charged. After 20 minutes game time and about 10 critical hits, it was dead, but i was bleeding and i had no soldering iron to cauterize the wounds, and a pack of hungry Coyotes were charging me. I killed them all, but i bled out on the floor about 3 minutes in game later.

It's weird playing Battlefront II after spending a while with BF1 since vehicles have been so de-emphasized. It sure makes Geonosis a pain in the ass though now that the AT-whatever isn't an all-powerful killing machine as it was in days of yore. I don't know what it is about that map, but the defenders always wipe the floor with AI attackers on that map.

Met an ore golem. Party is dead in 2 seconds.

I reload after altering the save to max out my main characters stats, plus pumping tons of edited-in character points into HP. Still an immediate total party kill.
What the hell kind of ore are these things made of?! Fuckyoutanium? Every other enemy in this place is easily survivable.

Plant yourself near an ammo droid. Keep it repaired. Drop medpacks to heal. Reload medpacks from the ammo droid.

If you are very careful, it is possible to take it down with only three rockets. 
But yeah, the only thing that keeps me from rage quitting when facing those things is that they give no long term damage, and they are almost always by themselves.

Did anybody ever figure out how to use the Snarks in HL 1? I never really got how to use them.

Also: even if you are Boba Fett, charging into a group of eight guys with shotguns is not a good plan.
