Despite wearing an accessory that supposedly prevented it, my girl-with-a-sword got confused and promptly murdered her two spellcasting friends before being finished by an orc.

Near-death experience, not sure if it counts: I was wandering around in the dungeon and stumbled upon a evil red-eyed baby with bat wings. It flew over to attack and I had to kill it.

About 3 levels later, I stumbled upon yet another baby that was exactly the same except the skin was very slightly tinted green. I said "Oh, I'm probably gonna get poisoned." It hit me, did 1 damage and poisoned me. I hit back, killed it and it exploded. exploding babies. Luckily, I had a little bit of HP left, the poison wore off and the explosion ripped any other enemies in the immediate vicinity to shreds. YEAH

Solar Flare from hell took out my Life Support, Shields, Engines, and worst of all, Door Controls by flooding the ship with fire. My 2 crewmen didn't stand a chance.

Noticed a bulging door getting bigger and bigger as something is pounding it on the other side. I prepare for the enemy, but suddenly the door blows out, hitting me in the head and killing me instantly. Goddamn vorts.

That freaking 'The king and I' mission. If the ai doesn't derp up and some how screw itself over before you can lure them back to the ambush, you're getting knocked all over the place by 3 different cars filled to the brim with gang members packing machine guns because you have to drive like a granny just to get them to follow. Saints Row 1

Chased down the rebel flagship, laughed as I spaced their boarding parties, stopped laughing when they sent over a android deathbot.  My crew of Engis finally wore it down in Medbay, but while I was distracted they launched what I'm calling "danmaku plasma".  My gunner Lauren EMPed their cannon half a second too late.  My token human (with a raging deck fire behind him and a slug for a copilot) managed to avoid most of it... but not enough.  The duct-tape-and-chewing-gum finally gave out.

On the other hand they had out of control deck fires and no life support, so I'm considering it a favorable draw.

My Kestrel and its crew of five were just two jumps away from the federation fleet when a Mantis ship attacked.  The Kestrel won the battle, but the only crew member left alive after a bloody skirmish with boarders was the pilot. Hull critical, life support down, door control down, shields down, medbay down and the rooms between life support and the bridge were all open to space. The pilot just barely managed to limp through the last two jumps and transmit the crucial data and the schematics for the stealth cruiser to the flagship, before the last O2 ran out and he suffocated at his post.

I'd like to think the sacrifice of my brave crew turned the tide of the battle, though I never got to see the outcome.

After constant fighting against rebel drones that had lasted for about 5 minutes, after I had taken out the flagships weapons, an assault drone manages to punch through my hull, straight into the ships life support. The life support is empty of air, my two soldiers are out on the flagship trying to take down their drone controls, all I have left on the ship are my pilot, shield specialist, weapons specialist and engine master. All engi, none with any combat experience. I send two in to try to fix it. The half dozen drones whirring around me slowly but surely take down my shields and...

A rebel ship jumped away with my crew, moments before I could tele them out.

To be fair, I wouldn't have survived the next encounter, had about 5 hullpoints left.

Ion storms and fire-causing beams aren't really helping when you try to destroy the enemy before it destroys you. The engine specialist ran through the burning and oxygen-deprived rooms (I was trying to vent them) to the cockpit and tried to flee, because all four other crewmembers were dead by now. He got there with just few health points left.

And just half a second or so away from being able to jump away from the combat, laser beams pulverize the ship into pieces.
