I got my head bitten off by a Giant Ant Soldier when I was trying to kill the Queen Ant, and plant the C-4 to blow up the nest.

I had no rice balls and was electrocuted by a giant spider-like creature with a satellite  dish implanted in its abdomen.

Spectrobes: Origins. Rice balls are the game's healing item and I somehow ran out before the boss battle. Miraculously, I managed to take it out the second time with no losses.

Killed by a large horde of Drifters, while riding my Monster truck. Stupid things just don't die instantly with a single volley of homing missiles.

Down to my last guy, who's a Shotgunner. Thought the enemy Rocket guy was gonna shoot a different spot, so i send my Shotgun through a building to try and flank. Prime the move, outcome plays out, turns out the enemy Rocket was set to fire at the same building i sent my Shotgun guy in. Cue explosion and defeat.

Playing Dungeons and Dragons Online. Fail three spot checks, run through an ambush point. Fail two more spot checks, run into the next ambush point. Five arcane skeletons start fireballing me.

I got a bullet right through the skull when I was fighting a group of raiders. Knew I should have took cover.
