I was fighting Enclave and winning, then the car next to me exploded from being hit too many times.

I got hit a by a car that threw me all the way to the statue of happiness and I hit a ground and died.

Crawling through a lair full of sleeping bloodsuckers in STALKER Call of Pripyat. It's hella funny when they all bum rush you and the guy leading you screams run just before getting swamped by bloodsuckers.

I had a nice Troll Monk going (Crawl) when I happened upon Rupert in Orc 3 who immediately confused me.  Behind Rupert was Aizul and Jozef.  I had two turns while confused before I was put to sleep and hit more a few times until I was dead.

Was following my Heavy around overhealing him while we wait for me to charge up. Just as I'm about to plunge us both into the midst of the battle, I get shanked in the back by a bastard in a mask.

So apparently a tiny Teir 1 airship with no maneuvers is no match for those pesky pirates. And they really like to steal your cargo. Do not like this server reset thing xD

I got trapped in a room with 2 Blood Mages, adn 6 or 8 Mercenaries needless to say after two of my guys went down the other two followed suit.

Halo: Reach, near the end of the second-to-last level, was trying to use a Ghost to help grenade-jump up to the Mass Driver early, but i underestimated how damaged the Ghost was, and the Ghost exploded from the grenade exploding, killing me with it.

Note to self, take a pristine Ghost next time i try that (not counting shooting the wings off so i can fit through doors).

I'm guessing... Mount & Blade: Warband with Native Expansion mod. I can't remember if Sword Sisters were in Vanilla.

They most definitely were. They're just obscenely tough to acquire (find a peasant women as a bandit prisoner, then recruit her and painstakingly train her. Or take her prisoner when attacking a farmer group and recruit from prisoners, then ditto with the training). In fact, given how bloody tough they are to get, I find it easier to imagine sacrificing oneself to rescue one from combat in Vanilla.
