Happened to my brother, not me, but regardless.

There's a particular place, where you can pay gold in return for level-ups if your party members have enough experience. However, you can also fight the monks who do this, in which case they'll either kill you (and then resurrect you) or do it for free (if you win). They're absolute beasts in combat, though, so we've developed a specific tactic revolving around turning them against each other and then killing them, both with massive area spells.

Party enters arena. As soon as all four have filed in, my brother hits the button to cast a magic immunity spell on them before the runemaster opens up with the damage.

Half of the room is enveloped in a massive explosion, the runemaster dies, and the rest of the party is below a quarter health.

"Wrong button."

I was playing with some Mystcraft stuff and created a world with liquid metal seas. I decided to jump in to see if it was dangerous. As it turns out, yes, it's a bad idea to jump into molten metal. Incidentally, I hadn't placed my book to get back and now I'm stuck on a 5x5 cobble platform in the middle of a sea of molten metal.

Beat a few missions in a row without the recommended suit or weapon upgrades. It got to the point where I could hardly do any damage, not that it mattered because just a few seconds of fire would eat my entire health bar.

Robocop. I'm surprised this game is still up and running, given the film's lukewarm reception.

Not really a death.

Tried to build a basic tower. After some difficulty, including a part that managed to shear itself away somehow, I get a not-crap-looking one up. I save the blue print, and spawn another. That's when I notice that a few blocks on the underside were not placed. I try to fix without the whole thing collapsing(happened with copies a partially off the ground to get at the missing sections). I give up when one of the fixes are considered separate from the main tower.

Strongly implied the judge was the murderer.  Ate a double-penalty that drained almost the entire health bar.  Followed up by accusing the prosecutor, which got me whipped under the stand.  I knew it was incorrect going in, but I found it quite amusing when the options appeared.
Admittedly, lots of things are amusing when you're on a combination of cold medicine, too much sleep, and not enough sleep.

Considering how many attempted-framings the Judge is practically complicit in...  You weren't wrong.
I really got to play that again.

FYI JFA is the worst one. Apart from the last case which is actually really quite good.

I'm actually kind of angry right now. Playing Xenonauts, Ironman Mode to stop me from wussing out when stuff goes wrong (In theory). I've been doing alright, shooting down many UFO's (although quite a few escape because four show up at exactly the same time and I only have enough craft to take down two) and succeeding in all the ground missions, losing on average one Xenonaut per mission (Not good, but could be a lot worse). First terror mission of the campaign (I have played them before) - I get a Private to try and throw a grenade at the first Sebilian I see. "No blocking markers? Surely that's going to land where it indicates." End of the turn, Private vaporised by own grenade. The following turn an experienced soldier fails a morale check after getting hit from the fog - okay, bad luck, but it happens - drops his weapon, and runs IN THE DIRECTION HE WAS SHOT FROM. Groan! Then about 4 Sebilians and 2 of the Chrysallid things showed up with a Zombie. After missing a 95% shot with my Sniper at the Chryssalid that popped from the Zombie, I Ctrl Alt - Del'd.

Spiders. Everywhere. Two giant ones and about 100 tiny ones. When they're not busy crushing you underfoot, the big ones lay eggs that spawn more teeny ones. And the teeny ones slow all your movement when they bite you.

Slowed movement + continuous scratch damage + giant spider beatdown = death. Lots of death.

Stumbled upon a Draugr Deathlord, which I am still nowhere near being able to beat.

Skyrim. I seem to have extremely poor luck when randomly wandering in to dungeons.

There are three or four guaranteed-spawn Deathlords, and I think there might be a Death Overlord in this one dungeon. You probably ran into one of those.

Heiterverzweiflung. Not a legit German word so much as something a friend and I made up in German class once. "Carefree despair". When life is so fucked that you can't stop laughing.

How else do you die in ARMA 2? An APC suddenly arrived, dropped off a load of infantry, then proceeded to drive through our formation and gun down anybody that wasn't fast enough to get away.

ARMA 2. It's better than when I first played it, though still not as good as Operation Flashpoint.

I embarked on the mission of slaughtering a group of goblin marauders for a village. I snook into the pits, only to find myself in the middle of a 2/3rd through riot. The mounts had broken free and were slaughtering trolls and goblins alike.

I snook throughout the edge of the cavern, apparently unnoticed, silently slipping my blade into the spines of unsuspecting victims, heading for the Goblin Lord of this place, and torturing my disabled victims.

Until a troll spotted me and charged. My incredible dexterity had me deftly dodge the attack. And that was the end.

Accidental friendly fire with a generic missile-damage. Accidental friendly fire with a dragon wand. Accidental friendly fire with a missile-damage-surround-damage. More accidental friendly fire with a dragon wand. Swarmed by kobolds. Hit by surround-missile-surround-disrupt (!) by enemy spellcaster. Accidental honourable seppuku with a generic missile-damage.

Edit: accidental TPK friendly fire with a dragon wand. RIP.
