Middle names.

As military dwarfs gain experience, sometimes they obtain a middle name.

What (specifically) triggers the gain of a middle name ?

Killing a lot of enemies ?
killing a lot of powerful enemies ? Specific type ?

Also, it's not a middle name: it's an extra name at the end.  "So-and-so . . . the Slayer!" only in dwarven form.  And usually less logical.  Therefore dwarven.
