Mayor hasn't noticed his demand being met.

He wanted a zinc door to his bedroom.  He has a zinc door to his bedroom now.  Is he just being a pain in the ass or something?  Which AI quirk is this and how do I fix it?

Are you sure the defined bedroom zone actually includes the door?

Maybe it has to be in the bedroom instead of on the edge?

In that case it might also help to set it as an internal door.

He doesn't want the door to be on the outside where any common dwarf can see it.  This is HIS zinc door, for his use alone, and has to be completely within the room.  You need to build the door inside the room, attached to one of the room inside walls, and then set the door as 'internal'.

Well, I suppose I shouldn't kill him just yet.  So far he hasn't been too bad.

Just the occasional export ban on hoods and a lot of mandates for traction benches.  Which helps me finish my hospital.  Once my hospital is finished though I'm just going to start making them out of zinc [his favorite metal] and then giving him a zinc tomb and then when he really irritates me bury him with a bunch of zinc traction benches.

You could also probably leave the door on the outer edge of the room, but leave it set to "internal" so that it's considered a part of it.

The same problem occurs when a noble demands a window in their room.  I had a baroness in a 40d fortress who demanded a clear glass window in her tomb.  Now a window in the tomb wall might have made sense in some bizarre, macabre way, but she insisted on having it installed inside the tomb, where the only thing you'd see through it is the other side of the tomb.

Demands don't "fail" - the noble just gets annoyed as long as it isn't being satisfied (i.e. no punishment), and after a while they just forget about it.
