They just constantly kill their subordinates for the most minor of mistakes or sometimes things that aren't even their own fault and never reward anyone...

If Batman is an effective template, the answer is significant amounts of "hero" induced brain damage. It starts as a little petty crime, and then you have your skull spiked into pavement and things just kinda' go from there. Eventually following the guy that's murdering people left and right for trivial mistakes seems like a good idea. Also your best friend is an invisible six limbed ostrich and you probably fornicate with brick walls in your off time. Cranial swelling is a hell of a drug.

I think the worldbuilding answer is usually "something offscreen". You generally don't see payday, or when the big bad captures a mook's family, or whathaveyou, on screen. Maybe there are actually crippling societal issues that the Big Bad is doing something about (and/or taking advantage of), but the story doesn't bother to touch on it. The list just kinda' goes on.

Oh, yes, Riot, you totally came up with the idea for Teemo. You didn't steal the idea of a furry little animal who sets traps and becomes invisible when sitting still from anywhere, no, not at all.

It was on a ~2 year break, but the author posted a ~9,500 word update this April. It's not dead, just very slowly updated.

Riot's Pendragon took down the Dota-Allstars site and replaced it with an advertisement for League of Legends, promising to put up an archive in a few weeks. A few years later, he actually finally put up the archive, with the suggestion forums taken out to hide the fact that Riot stole some hero concepts from it.

They still claim that they came up with the ideas for certain champions that were taken from the forum themselves.

Don't forget that we usually see them when things are falling to pieces. We don't see how things went when the Evil Boss was on top of the world, and their subordinates basked in the reflected glory and skimmed luxury of the boss's leavings.

This morning, instead of removing my pajama pants and donning normal pants, I just put on my normal pants on over top of them. I did eventually realize this, halfway through my morning routine, and removed the extraneous pants. I was somewhat surprised that I didn't even notice the extra pants. Further experimentation revealed I could stack about three pairs of pants without noticing much extra bulk. Any additional pants significantly inhibited mobility.

That is all of my wat. I can't put on ONE pair of pants without noticing a loss in mobility. Curse my stupidly thick legs.
