I just found a sword in Caves of Qud (a roguelike game) that requires four hands to wield...

My favorite build is a summoner build, where other things fight for me.  Clones/charmed critters, and plants.  I sit in the back with a 2h sword or mace and focus on sniping problematic mons

There's a sword in Munchkin which requires three hands I think, and has a huge bonus because ways to get extra hands are fairly rare.
