There would probably be some silo explosions (almost certainly conventional, but you never know, there was that time the Air Force was one impact trigger from nuking Goldsboro, NC), but the more likely failure wouldn't be mechanical. It would be human. Not everybody is going to willingly usher in the end of civilization, and if you're going to die anyway taking drastic action to prevent a launch is entirely within the realm of possibility.

I think some declassified Cold War document said that they were worried up to 20% of launch commands would be defied either passively or even by force.

For an assignment, students had to do regex's. One student's work was just a train wreck of nonsense that did not accept anything and was completely broken and non-functional. They were convinced it worked when they tested it, well "kind of"(?) anyway. They got 0 and they proceeded to blame everyone else but themselves. They are incredibly stubborn and don't really seem interested in learning.

Similar things have happened to me before. I once ran into the legendary Shook on the Toribash Forums shortly after he had achieved greatness there, I've seen MetalSlimeHunt in youtube comments, and some other Bay 12 user (I don't know what their forum name was, but they clearly recognized me) ran into me in the same place.
