Dorf doesn't want to be healed?

I have one dorf crawling around the fort with his guts trailing behind them (if that's what those 2 grey tildes are following him onscreen). He's been to the hospital but the chief medical dorf let him go, and under his "Health" tab it says "No health problems" even though his guts are dragging behind him. Is this a bug? Or can a dorf choose not to be healed?

I go to hospital, guts hanging out.  Doctor give me stout dwarven rum and clean bill of health.  Like good dwarf I go back to work.

Such is life in the Fortress.

Perhaps they didn't get stuffed back in when his wound was sewn shut.
