I'm going to try to be as non-explicit as I can here. I know that this doesn't count for a whole lot on the internet, but I'm almost thirty, and I've used...well...let's call it an inordinate amount of them. The condom isn't a terribly complicated device, but like anything else, it does have optimal operating conditions.

A) Don't store one in a wallet, or pocket. Heat and constant flexion wears them down and makes them brittle.
B) Don't stretch them when you put them on. They are easy to tear if you pull hard. Leafsnail is correct, you have to roll them on. Gently. And there's nothing wrong with the woman doing it, it can enhance the experience greatly.
C) If you're using a lubricated one, the lube goes on the outside. Duh. If you aren't using a lubricated one, you should probably get a lubed one (or lube up), because they're not very comfortable for either party when they're dry.

Just...be careful when you use them, and you should be okay.
