Attempt to build a teleportation-based rail-gun.
If teleportation can deform a surface it can compress a rail, forcing the contents to progress along the rail.
If it can generate or displace matter, then it can increase pressure in an enclosed space, propelling something along a sealed rail.
If it moves material, then perhaps you could perform partial teleports or unstable teleports to make a static state unstable, causing an object to accrue velocity as it is teleported along a rail.
Maybe you can edit velocity directly.

Construct a missile, use a segmented worm as a base.
Have the forward segments hold various corrosive materials with razor-shaped seeds and/or spores embedded in the skin.
Have the rear segments hold massively pressurised liquids and shaped to maximise energy transference.
Once launched it will release its segments one at a time, the segments, once released, will propel the missile, which will have sufficient flexibility to steer itself. It will tend to take an indirect course and continue accelerating as long as possible so as to have a less predictable course. Once it makes contact with a target its porward sections will be destroyed, the seeds are intended to pierce a chemically unreactive covering to maximise corrosion to armoured targets. Any surviving seeds will attempt to sprout, using the remains of the missile as food, and deform the surrounding structure as it grows, preferably it will launch projectiles at any nearby objects.
The intended function is as a homing missile that will evade point-defences, produce moderate damage to a resistant target, and to create an ongoing threat, potentially disabling a target after a battle has concluded.
