My duchess died for unknown reasons, and when I got a new duchess the next immigration I then had two mandate-making Duke Consorts.

The same thing happens if the Consort dies.  A new Consort will come, but attached to another Duke/Duchess.

Easiest way to get rid of that? Kill him.

Well...  what should happen to him?  Should he leave?  Be demoted to a regular dwarf?  Go mad and die of grief?

Demotion. He's a "consort" not a noble persay so when he's not "consorting" he's nobody.

Well... Perhaps she just has more than one consort. Its not like polygamy is all that uncommon among individuals who have the power/freedom to make their own rules.
