I actually dislike count only starts. Takes so long to grab good land compared to big kingdoms and duchies. I guess an all duchy start would be pretty awesome.

How do you take in-game screenshot? I'm currently getting ready to smack the Aztec, and I want to screenshot the incoming battle.

F10 and F11 for CKii (and most Paradox games) in particular, and F12 for Steam.

MyDocuments(or user?)/Paradox/CrusaderKings2/screenshots.

Saves are also somewhere in there.

Well I'm not too sure about it but I suspect a perfectly stealthy murder will appear as "natural death". Either that or there's been some weird coincidences  :P
Nope, murder is always a suspicious accident. Natural death is... unknown weird game causes that everyone complains about and Paradox doesn't seem to know how to fix.

Natural death isn't really unknown, characters are born with a hidden health stat. Characters with below average health tend to die young from "natural causes".

My vassals tend to die as maimed cripples.

The normal max age for people in Middle ages was somewhere around 35-40, IIRC

I can't be arsed to find the source but basically this is a bit wrong. The median age during the middle ages was low yes, but that is not because people didn't live long, it was mostly because of high infant and child mortality rates. If you just got past those first few years you stood a good chance of growing old and wrinkly.

As long as disease didn't kill you, which doesn't seem to happen enough in this game.

And by old and wrinkly we mean to sometime in your 60s. After that the life expectancy drops off a cliff again.

A 22-year-old dying of natural causes wouldn't be unheard of. "Natural causes" includes all ails and diseases that isn't worth remarking on. Including such things as appendicitis and other internal inflammations and infections and such.

Okay, I'm trying to reorganize my Empire to make it more manageable, is there a reason why I can't transfer vassalage of a Duke to a King?

I think If they have land outside of the de jure kingdom you are trying to give it to then they can't be transfered. They also can't be transfered if they're in a civil war of some sort.

I found this mechanic to be a huge annoyance. I'd have Kings with multiple -25 relationship malus towards me because they wanted control of a duchy I couldn't trasnfer to them.

You can always just destroy the title.
I personally never have king vassals as an emperor. They can cause all kinds of trouble for you. I usually just destroy as many duchies as I can and try to have counts everywhere. The 2 county duchies will almost always become a dukedom but you can't avoid that.
