Not necessarily the worst injury but kind of funny...

While fighting a human siege, two of my Axe Lords were mauled by war Giant Lions. One lost his right arm at the shoulder, the other his left. I don't think they ever even bothered with the hospital (except Lefty went to get sutures for a cut on his LEG), they just finished the fight and went back to sparring in the danger room.

I can just see that hospital visit...

URIST McLEFTY: Stitch up me leg, doc.
DOC: But... your arm's off.
URIST McLEFTY: Giant lion took it.
DOC: Did you recover it? I might be able to --
URIST McLEFTY: Wasn't much left after I pried it from his jaws and beat him to death with it.
DOC: ...just the leg, then?

I had one miner get knocked down an 11 story deep pit by a cavein.  He lived with every bone in his arms and legs broken in multiple places.  The doctors even managed to piece him back together and he was back on his feet for a few weeks.  Unfortunately, he acquired an infection and passed away about a month after recovering.

A copy-paste about a human in adventure mode, but quite fitting to this thread:

I tore out a mans skull. Strangely, he survived. I went on my merry way, certain he wouldn't survive LOSING HIS SKULL.

He did. I found him some time later, with a "Massive round scar" on his head. And no skull.

NO SKULL.

You know, in that CRPG Dragon Age Origins, dwarves have a very terrible (and by my opinion awesome) way of healing wounded dwarves(but mostly the procedure is done on healthy dwarves). They'd carve/forge a hollow stone/metal casing, lay the dwarf in it, close the exoskeleton, and pour molten Lyrium(magical metal with hallucinative/mind controling powers) into the openings, until the screaming and struggling stops and the liquid metal cools down and solidifies. Then the exoskeleton would come to life, as a golem.

I'd very much like to do exactly that in my fortress. Of course, for such a system to actually be feasible DF still has long ways to go... but now that long, methinks.

Should have beat him to death with his own skull.

Smithdorf who had an arm mangled and a great huge slash across his belly with his guts hanging out. He got struck by a Fey Mood and went to make ... something or other. I lost track of him at that point (lots of dorfs and animals running around) and last I saw he was walking along with his intestines dragging along behind him.
