The problem though is defining what is home turf for some of the combatants.

Dr. Clef would be Site 19. Luke would be either Tatooine or a Rebel Alliance base.
PS would be his imaginary world place, you know, the place where basically all of Problem Sleuth occurred. Tex would be blue base in either Valhalla or Blood Gulch.
Sollux would be the troll world or something, I dunno.

Having them fight in their home field might not really be all that fair. Some of them would be almost unstoppable there, while others would be no better off than anywhere else. Ex: Dr. Clef would be practically impossible to kill in Site 19, while Isaac has no real advantage anywhere in his world(well, unless allies are allowed, in which case his hometown could collectively kill anything you could imagine).

Neutral ground is fine. Home turf is way too complicated and annoying.

Theres also the advantage of knowing his/her home turf.

I would say maybe a 'neutral ground' with various obstacles, random crap, and destroyable objects.

Well, opinion seems to be unified on the idea of injuries NOT carrying over. 
The battlefield will be some unspecified place that gives no undue advantages or disadvantages to either combatant. 
The main issue left to resolve is:
Will the combatants know they will be getting into a fight?  Will they know who they will be fighting?  How much time, if any, will they have to prepare before the fight?

The combatants shouldn't know when or who are they gonna fight, and should have ample time to prepare.
