Since whatever the obvious 110-Montauk is, canonically is less severe than what it really is, the absolute worst thing I could think of is that she enters labor each day and the D-class personnel literally have to rape the abomination back up her birth canal.

Usually.
If needs are dire they can be criminals not on death row to ordinary civilians.
Although it may be possible they are clones, considering the foundation had success with cloning technology.

That would be the first recruting layer, yes. In the event that those run out, personel may be recrutied from lower prisons or the general population. Monthly full Class-D termination remains the same. Individuals within the Foundation may also be demoted to Class D as a punishment.

They still have the Cthulhu locket, which does the same thing basically, if in a slightly more dangerous way.

It's a pendant that projects healing rays. It was made from a pill of 500 placed into 914 and put on the fine setting. It does a little bit more than heal, however.
