Age of Myth- lots of titans/megabeasts
Age of Legends- some titans/megabeasts
Age of Heroes- something to do with the ratio of titans/megabeasts to legendary combatants
Golden Age- all civilizations on equal standing
Age of Dwarves/Elves/etc- one dominant civ
Age of Twilight- most civilization has crumbled
Age of Fairy Tales- almost entirely human world, perhaps with a cyclops or two and some kobolds
Age of Death- almost everything is dead
Age of Emptiness- everything is dead (I've heard of trying to achieve it, but I've never heard of anyone actually succeeding. Could be a rumor)
Age of <historical figure>- one dominant titan/megabeast (possibly one extremely powerful hero, too)
Age of <histfig> and <histfig>- same as above, but with two
Age of Three Powers: same as above, but with three

That's everything I can think of, plus requirements.

I think I once genned a world that was in the age of emptiness.  This was a pocket world with extremely low temperatures, though.

The Age Of Emptiness can happen, but almost never without tweaking worldgen into a death world. You can't have it in fortress mode either, as your dwarfs being alive changes it to The Age of Death/Twilight. Although, I think that the FUN in .31 makes this impossible now.

the FUN doesn't count towards the amount of things and shit still alive

so you can have a age of emptiness while Fun Is still there

From what I can tell, if all megabeasts are artificially killed, it goes to Age of Twilight.
I'm pretty sure that only happens if you modded in vampires...

Apaarently if you kill them ALL then it is the age of urrist Mcbadass. I once settled somewhere with my team of dwarves and it was the age of the hydra and ocean titan but then when i settled. It sounds weird but, my cart had driven over the hydra and it became the age of the ocean titan. 8P

age of civilization is when there are either no or only 1 or 2 (not quite sure) fanciful creatures or nothing but mundane creatures
ive gotten it a few times with a modded game

am pretty sure there was a research thread on it a while ago

You're thinking of the Age of Fariy Tales. It involves having nothing but mundane creatures, mostly humans, and only a few fanciful ones. The Age of Civilization is for when all existing civilizations are growing and no other age has been triggered.

This thread inspired me to try generating a pocket world with low temperature.
As you can see, I highlighted in red an important numerical figure which suggests that I must create an adventurer and kill everything that lives in this realm.
You will also notice that the world is currently in the "Age of the Hydra and Hill Titan." This must be rectified as well.
