Psychiatrist are medics as much as your regular surgeon and can prescript you with medicines or operate your appendix in case of emergency as good as say, a dermatologist can.
I don't think I would want to trust a psychiatrist or a dermatologist with an operation on my appendix.

If the alternative was a painful, drawn-out death?

In what world are your choices:

a) a long painful drawn out death where you don't have time to find a real practitioner of physical medicine

or

b) that your only medical options are specialist fields like dermatology or psychiatry.

That sound is the sound of a million doctors screaming for blood.

And as for scenarios, pretty much anything. You went camping with a friend who happens to be a, say, dermatologist. Appendix did not enjoy the scenery. There is heavy snow, and your neighbor is practicing psychiatry. Or yeah, actually, something like a war.

It's conterfactual anyway, since you might as well complain that since you DON'T have appendicitis right now, why are we even talking about such a possibility?

Also, all doctors, no matter the specialization, have to know stuff. A surgeon is just an MD that specializes in doing surgeries, not one that is the only one QUALIFIED to do so.

No, the vampires have all the blood.
Unrelated, but it's funny how stealing blood from blood banks is considered the moral alternative to bloodsucking in vampire fiction, but no-one ever talks about what happens when most vampires are stealing from blood banks and people who are meant to actually have the blood... don't get it, cos the vampires are stealing it.

There is a surprising amount of blood that just gets thrown out after it "expires", especially of more common and less useful blood types like AB+. If they were ok with drinking less useful blood typed blood that was nearing the end of its shelf life then they shouldn't be causing any real harm to anyone.

(The reason there is a constant need for blood donations isn't necessarily because we are using all of it up, but is rather linked to the fact that we like to have at least some of each type in case we need it for an emergency, and the shelf life of blood is rather short [6 weeks]. This then creates a constant demand as old blood expires, leading to the constant need for new blood donations.)

The six week limit is for red cells or whole blood, I believe. Plasma can be frozen and lasts for a good year, while platelets are only good for about a week.

It sure is a good job there's enough blood to go around. Wolfram reckons it's 1.3 times the volume of concrete used to build the Three Gorges Dam. So I guess if you wanted to make a dam from congealed blood you could. Only requires killing the vast majority of all humans.
