Because really. Medical history has gone through a . . . lot of dark times to get to this bright future we're arriving because of assumptions via observations. Like Phrenology. But that's assuming dark = Chaotic and nothing good came out of it. :V

... and yet, the latter arises from the former. It's not synonymous, no, but it is an explicit consequence. As noted, it is, indeed, a laughably simple surgery that results in significant personality shifts.

It's just, uh. Not very precise. Or generally desired. And the particular method mentioned would probably kill you. But it would do what Puff said it would do.

Pretty sure we've got less hamfisted processes (beyond lobotomies, mind) as well, though. Still very rarely used and far from particularly sophisticated,* but they're out there.

* And, considering the variance between human brains, even if we do end up being able to perform tailed personality alterations, it's probably going to be a long time before it can be done on anything but a subset of the population. And prohibitively expensive for anything but limited use. There's much more simple and economical ways of population control, anyway. Propaganda, just to start.

So, for what it's worth, trans* (by my understanding) is about how you identify yourself, and orientation is about who you find attractive. There's probably some interesting interaction here; a trans man (physical woman) might be gay (attracted to men), and might not be sure how to handle another trans man (physical woman). It would probably depend on what characteristics of men are actually attractive to that person, and there's no Right Answer for what those are or ought to be.

Now, I get that gender identity might not seem important to you, but people have all sorts of weird ideas about what defines them and as a general rule we respect those. Like, religion is unimportant to me, but lots of people identify as holding religious beliefs, and that's OK. You pretty much go, "Yeah, alright, that's a valid thing to care about", in the absence of a very good reason to do otherwise ("Civilian Kill Count", for instance). But that exception should really go without saying - most of the time, there is no such reason against a perspective, and so most things are respectable.
