I always give my chef medical dwarf an office and bedroom, which is as good as any nobles room. I also put another room, filled with beds, as a dormitory(not really hospital, but in it), for new migrants. I smooth all the walls, and my doors I all dig out 3 wide, then build a wall in the middle different colours to code my rooms. Usually red is for patients, blue is for doctors, and I dont include a well, but I have a small fresh water pit nearby.
