You need to build stairs/ramps where there is nothing to dig into (IE, you're building it in an open area, not digging the stairs into a wall.) with the (b)uilding -> (C)onstruction menu, not the (d)esignation menu.

If you're roofing over a protected area outdoor, I would recommend building a ramp on the outside of your wall (possibly inside a temporary wall if you're worried about invaders).  That way your roof won't have any holes, and you can remove the ramp from ground level (without that annoying 'down stair' hanging off the edge of your patio roof forever and ever).

I try to use ramps in my vertical constructions as much as possible because you can remove them from ground level and leave a clean structure.  (or just build a stairstack near the vertical construction, link to the construction with bridges or grates, and sacrifice a brave dwarf to deconstruct the bottom of the stairstack thus collapsing the whole thing - and crushing the poor dwarf).

I use ramps and whatnot to build up a level, and then use bridges to make catwalks as scaffolding.

Note that you want to be careful when taking them apart; and probably not use wood, as dwarves may be collecting the wood when the bridge under them is dismantled (that's how the only two dwarves to die in construction accidents died).
