I recently started on a really good map. Plenty of coal and iron and so on. But, my dwarves saw fit to park on a frozen river. The river even had little anthills all over. After about 5 seconds the river unfroze and my dwarves all drowned.

Suggestion: Make it so that dwarves do not start on frozen water.

Unless they're on a glacial map where they have no choice.

But then, I've had problems with glaciers in that building stairs out of ice seems to have a 75-90% failure rate.

I'm just avoiding ice maps now until ice stones are properly handled. cant store them and cant reclaim them if a army of zombie polar bears killed my first dwarves off. Unless someone made a mod?
