I think one of my starting 7 will have full points invested in to surgeon. Unless I am just messing around.

Unfortunately its not that easy. There are 5 different doctoral skills: Surgeon, Diagnosis, Wound Dressing and Bone Setting.

I think that surgery and bone setting deserve more point allocation than diagnosis, though, since a failed diagnosis may not be as critical as a botched surgery. But at any one point during the healthcare process, something can go wrong via lack of skill.

Oh, yea. I forgot about that side affect, but maybe they will be slightly more useful that the fish dissectors and soap makers.

What I'm looking forward to is creating a system to train the medical skills. I think my bonesetter will require a lever in his quarters to the retractable bridge over the 4-level drop... and random peasants will be recruited and given masterwork steel axes with no body armor

Remember that the sparring system has completely overhauled to reduce sparring injuries. I am sure though that you could do this too, but it may have a negative affect on whoever is in charge of military academy, training and recruits.

 Seems a bridge droppping device would work fine for most small injuries. Broken bones should not be too much of a problem.

 Although the splattering of blood everywhere in an attempt to repair a compound fracture worries me.

Perhaps wooden spike traps on slow automatic repeat throughout the fortress? I'm sure I could make it produce moderate wounds at a regular interval without excessive casualties. That would make for surgery and wound dressing. Should start a challenge to see who can get one dwarf to legendary in all medical skills the fastest. Bonus points if you mod in butcher's aprons or white coats, and have him wear one and nothing else. Double bonus if you can manage to get all the rest of your dwarves incapacitated at the same time, with no deaths.

I feel bad for all of the migrants who are going to be mutilated for the good of Dwarven Medicine.  Oh...did I say feel bad?  I meant *evil cackle*

Honestly it's going to be fun, my first fortress I am planning is going to be a massive military academy, so besides having a huge population of soldiers, it is going to need plenty of good doctors...oh the carnage that will be had!
