I was not a kid anymore, but what also freaked me out were the ghosts in the Wheel of Time game, in the travel dimension. It's not too hard to stay moving and stay ahead but you can hear them whispering behind you...

I was curious as to what would happen if they did catch up with you.
I looked back.

Oh god, the whispers.

Oh god, the whispers.

The Black Wind? Lordy, that scared the hell out of me. That place was eerie even before the wind shows up: crumbling staircases and walkways suspended in a pitch-black void with silence everywhere, then you start to hear a distant breeze..... AHHHH! That game had a lot of creepy parts in it. The deserted city, with the mutants and the soul-eating tendrils of mist floating around, the Ways, and then there was that one mirror....that made me jump so high I almost fell out of my chair. the enemies were all so strong that it was always kinda tense, because some of the enemies could kill you really fast.....mmm fond memories of being chased around by trolloc chieftans. I wish there was another WoT game I could play. (no I don't count Dragon Age)

There were these facehugger thingies in Blood 2 that scared the crap out of me. You crawl through some pipes with maybe a foot of water in them, and suddenly you see a shadow moving towards you under water. Seconds later, the grossest thing ever is eating your face. These things were impossible to hit, too, or maybe I just sucked because it was my first FPS.

Basic premise: You're staying the night in a mansion. There are ghosts that jump out and steal your soul.

There is one ghost per material type- so, there's a metal, an ash, a water ghost...

Unreal 1, level 2 first brutes:

What about the first Skaarj?  When the lights in the hallway went out one by one you knew Something Really Bad Was Coming. 

Half life really did it for me tho.  Just walking by an air duct in any FPS now sets me on edge.  Ravenholm in HL2 didn't scare me ... til the skinless leaper zombies.  I just about jumped out of my skin.

In Resident Evil 4, there was this one room near the end of the game that, while not containing any enemies, had a dumpster that had a bag in it.

There was something in that bag, as it would squirming and writhing when you opened the dumpster. It scared the hell out of me, because I didn't know what the HELL was in that bag.

It didn't help that you couldn't interact with the bag at all, nor could you damage it in any way.
