Caring about your appearance isn't a bad thing.

Caring on a basic level ala "i guess i shouldn't be smeared in shit and coal today" is fine (a bit more than that is also okay, i suppose), i wouldn't call that narcissism or vanity. I may be an eccentric, but as soon as it becomes an absolute necessity to make sure you have your stunning good looks on when waking up in the morning, that's where i'd get worried. I don't shower and shave for the sake of appearance, but rather because my scalp starts itching and so on. It also has the added bonus of silencing those poor sods who can't find it within themselves to disregard how people look in favour of how they act.

I think there's a misunderstanding when it comes to people wanting to care for their appearance. Just because you want to look good and enjoy your appearance doesn't mean you're vain about it. Some people try really hard to look good, and even if they really do, still don't like the way they look. You see it all the time on this thread. It's not as if all good-looking people know they look great. Nor do they look down on other people if they are confident enough to be comfortable with the way they look.

Vanity, and on the next level, narcissism, is when you have to shove it in people's faces and tell people how good you look. "People simply must know how fantastic I look or they will die!" Which I've.. Never met anyone who was like that. Unless you're sociopathic all humans are self-conscious about the way they look. People like being liked, and appearance is a part of that.

It's a fallacy to think that completely disregarding your own appearance means you care about it more than the people who gel their hair or wear jewelry or makeup. Just like there's nothing wrong with someone who doesn't shower every day, there isn't anything wrong with people who buy and wear makeup.

All right, i concede that i may have spoken in rash terms. I don't think i can adequately explain what i mean or why i mean it, but in the simplest of terms, i just find it silly to make a fuzz over outward looks. I legitimately can't understand why others would make such a big deal out of it, which kind of makes me wonder if i have some mental disorder that i don't know about yet. If i have inadvertently offended or hurt someone, then i apologize for that.

What matters most is that you're comfortable with the way you look, which for most people means having some interest in one's own appearance.  This is only a problem if that leads to fucked up priorities, or unfair expectations projected onto others.... which, sadly, happens a lot.
