Generally my names are nice and boring, because I lack the memory for several names. However, sometimes I put in the effort for something more.

Generally this will either be a poorly disguised meaningful name (Like Pyrin for the Fire Mage) or, if I'm feeling really smarty, a very well disguised meaningful name, often with bible roots.

Sometimes I go with a He-man-ish combination of masculine syllables. Start with something like Va, with a good "v" like Vulcan or Violence. Then something scottish... Ness? Loch Ness? Yes. What's manlier than a giant icy lake in scotland full of prehistoric monsters? Then... godliness; specifically, a sun-god. Something that sounds like the name of some anchient sun god. Maybe Sa. Simple, guttural, almost... YES.... put those together and you've got yourself one manly name.

Va-ness-sa .... Yep looks good. Vanessa is a totally manly name.  ;)

Also now I wondering what the worst name to have is for writing it backwards.

Also, to get a fantasy name, just spell yours backwards!

For example, my middle name becomes Nahtanhoj and Henry becomes Yrneh.
Oh, my name sounds awesome with that theme.

Or my forum name
Rololtenti (Ro-lol-ten-TEE)

Behold, a new adventurer name: Naimad Rololtenti
Sounds Italian with Middle-Eastern influence; less Irish/Czech-rooted. Meh.

Alternative is also reversing phonetics/syllables.

Anmida (An-MEE-da)
Orlolnetit (Or-lull-net-EET)

Neat, now that's more like it. More names I can use. ;D

EDIT:
Try using those methods, but try to mix in your DF title along with it (so you also won't need to use your last name). I don't know if all of them would end up as awesome as mine turned out though.

Wait why not do both?
Now I get:

Ukalin Nepsric  (Reverse syllables then spelling)
and
Luknai Nepsrec  (Reverse spelling then syllables)

hmm, the last name on those two are very close.

Not bad, now I have 4 awesome names for adventure mode or any other ill-fated adventurer of mine.

Because I'm not very creative, I use Cheeetar or Hasuin if I need something that sounds like it could be a name.

Usually Myroc, unless I'm doing a female character, when I usually invent a name on the spot.

Oh man, this reminds me of my latest FFX playthrough, where I named Tidus as Fritolay (like the company that makes all the chips), then I named all the aeons after candy bars.

My favorite: I named Yojimbo "Pay Day". Oddly appropriate.

Or a combination of one name or another from movies like Die Hard and Dirty Harry and such if I'm in the mood.

Mostly it depends on the setting and character, but here:

Male

Gristom - For the sleazy rogue type or the CG warrior. Will always have a goatee.
Delgado - Good-looking antihero or villain.
Dargor - Legendary warriors. Bonus points if you know who I stole the name from. ;D
Benton - Young-ish characters. I stole the name from the Kay Scarpetta books.
Gantrithor - I'm fairly sure I stole this from Starcraft, but I'm not sure.
Tikboy/Jhun - Used in jokes. Jhun is a fairly common Tagalog alternative for John, Tikboy is the nickname of our former Mayor.

Female

Imber - Queenly types.
Jennifer - I always liked that name. Used it in two of my school plays.
Anariel - ELFS and angels of either gender.
Lala - Not sure, but I'm fairly sure I've thrown this name around in some stories. As a nickname. I think I stole it from Gundam.
Regan - Stole the name from Exorcist. ;D
Rebecca - One of my favourite Biblical names, and I used it a lot in conjunction with Jennifer.
Messina - A play on "Messalina". I use it in Roman Empire period situations.
Margaret - I use it about as much as Imber, for lower-class people.

Makes sense. You can't tell the difference anyways!

Hmm... reversing my first name and forum name gives me...

Sounds oddly middle-eastern.

Some other names I use:

Rogan - has a nice warrior-y sound to it. Not good or evil, sort of the sell-sword you'd expect to have this name.

Azazel - Used for wizards, it's based on the most mysterious of demons.

Maddock - For the shadier figures, rogues and the like.
