In the days when Sussman was a novice, Minsky once came to him as he sat hacking at the PDP-6.
“What are you doing?”, asked Minsky. “I am training a randomly wired neural net to play Tic-Tac-Toe” Sussman replied. “Why is the net wired randomly?”, asked Minsky. “I do not want it to have any preconceptions of how to play”, Sussman said.
Minsky then shut his eyes. “Why do you close your eyes?”, Sussman asked his teacher.
“So that the room will be empty.”
At that moment, Sussman was enlightened.

Did you turn off cave-ins in init?  I'm not sure, but that might be why.

I keep thinking glacier fortress would be fun, but it always ends up as cave layer fortress.

You're not using straight 40d16 - you're using the Mayday mod (or something else with the Economic Stones mod applied).

Also, the reason you're not getting a cave-in is because you missed (at least) 2 wall sections at the northeast corner, just at the surface of the magma pipe.

[edit] Confirmed - upon digging out those 2 tiles, the roof promptly caved in (and crushed a legendary miner).

P.S. If you don't get this note, let me know and I'll write you another.
It's amazing how dwarves can make a stack of bones completely waterproof and magmaproof.
It's amazing how they can make an entire floodgate out of the bones of 2 cats.

If the graphics set you applied is Mike Mayday's graphics pack, you should know that it also includes the Economic Stones mod, which renders saved games incompatible with unmodified versions of Dwarf Fortress (giving the error "Missing Reaction definition" and then immediately exiting whenever you try to load the game).

Oh, THAT'S what causes the "Missing reaction definition" error.  Good to know.
