That's one of the few "Professionalism" exceptions, mostly because nearly everyone respects the medical community to some degree. If, however, you run into somebody (call him Howard Johnson) on the street, or work with the same person in most settings, calling him "Mister Johnson" is an insult, in no small part because, 90% of the time, that form of address is used ONLY by persons that are placed above you (such as top-level bosses or police), usually when you are in some sort of trouble (being fired or arrested, for example). Thus, calling him "Mister Johnson" is the equivalent of "You are lower than the dirt beneath my feet,, and I OWN your sorry ass."

I wouldn't call anyone without a doctorate/M.D. by their title unless they were my teacher. There's not really any insult to it; it's just sort of weird and distant.

All my high school teachers insisted on Mr/Mrs/Dr Lastname until we graduated, then they didn't really care. The default for pretty much every college professor I've seen has been by their title in class, but they don't really care outside of class.

In this situation, I'd call anyone who is expounding on a new theory as "Sarkeesian" or "Anita Sarkeesian."

I use titles and honorifics for people older than me (respect), or people younger than me that I really, really hate (total anti-respect).  The difference is largely in voice tone.

Heh. I just endeavor to use names and titles -- explicit identifiers in general, really -- about as close to as little as humanly possible as I can manage, when actually talking to people. It's generally quite impressively little. Much easier to largely forgo doing such in person than online.

I just... sorta' don't like names, I guess. As a general thing. They're convenient but somehow... interfering? A lot of the time.

It's hard for me to attach much more meaning to a proper name than 'net aliases or just random number strings, some days.

For me it's at least as much because I have a hard time with names as anything else. It's nearly as handy a trick as the old one where you ask someone their name when you can't remember, and then clarify that you meant their last name after they give you their given name so as to avoid offending them.

There's also the matter of people who work in a chain of command. Certain individuals in my workplace are Captain or Captain [familial name], full stop.

I don't think that using "Mister" in a normal tone would ever be considered offensive. In these days it wouldn't really be considered offensive not to use it, but I don't think that it is all that bad to use proper etiquette, either. Usually I just use "Mister" and "Ms./Mrs." (in addition to sir/ma'am) at teachers and other people who are in some position of power over myself. However, I don't think that it would be offensive at all for anybody to use it with those at the same position as themselves.

Also, I was surprised and happy to find a museum in LA that was all about anti-psychiatry.

Then I found out it was run by Scientologists.  -_-
