So I've reclaimed a fortress that I abandoned 'cos all the dwarves were starving.
Things were going fine until I tried to make a magma forge.
b-w-v just doesn't work; magma forge just isn't present on the last menu list.

1) You need to have actually discovered the magma for magma workshops to appear in the menu.
2) If you used some sort of tool to reveal the map features, the workshops will not be enabled once you get to the magma.

I think it's a known bug with reclaims. Dunno how to deal with it though.

Bugger, that explains it. The magma pipe was already visible on the surface the first time I embarked. Must be that known bug then smjjames.

This. Also, there exists a utility which will allow building magma workshops, though it must be run each time you start Dwarf Fortress (at least until you've built all of the workshops you want).
