@Knigel  I'll second that it is indeed possible.  With my current adventurer (that coincidentally has managed to survive longer than any other adventurer I ever had, including the superhacked ones) a goblin wrestler had grabbed my helmet, removed it, and proceeded to try to beat me with it.

Naturally since I'm some kind of supersoldier slayer of gods, he instead got shown his place and I retrieved my helmet.  But it is most certainly possible.

I've had hags beat me to death with my trousers before. :/

It's not an embark so much as seven dwarves having a simultaneous strange mood and going off to build an artifact fortress that menaces with spikes of awesome and hanging rings of death.

one hand and arm, and one leg left to go! The head is the hardest. I suggest biting a balor, then jumping in a pond.

They don't seem to come off in .18. No matter how hard I try.

Double Fatality!

While I'm fairly sure that's impossible, I have headstabbed someone and then immediately counterstriked another opponent to death, before moving again. That's as close as you can get AFAIK.

Striking down one guy and counter-attack striking down another guy while only using one attack command is exactly how I completed that achievement.
