For the collar thing, it's around the creepiness level of wearing jewelery.

It may be more creepy depending on how noticeable the bell is, and if there's any inscription on the collar.

Creepy? Yes

You would be better off getting the girl it as a present if she likes that stuff. (I think Collars and Chokers are out though.)

I can imagine the pure opposite reaction. Especially if she just watched a "Women abused" Movie (Is there a correct term for this? Enough was probably the worst I've seen of this genre)
