I'm of the opinion the DreamJohn wasn't actually injured, and the blood was some sort of indication that the Quest Bed was fufilling its "purpose".
Notice how DreamJohn has no hole in his chest?

That would mean that she didn't die in Make Her Pay, but she probably wasn't going to last much longer anyway so it's not like it was that big of a deal.
I would think she probably had Tavros drag her off to the Quest Bed after Make Her Pay, and then bled to death.
