I once set up shop in some friendly goblin towers. As I decided I should keep the whole darn thing very safe, I figured I'd live in one tower for now, and wall off the bottoms of the other two, connecting them with bridges from above. Unfortunately, one of my masons decided to wall himself into a tower. After quite a while listening to "Urik cancels carry water: no buckets", or whatever that one message is, I realized he was trapped. I hurriedly released the now starving and thirsty dwarf, who stumbled his way over to the food stockpiles to get a bite to eat. He got his food, sat down and started eating. And promptly died of thirst.

Nice temperate map, froze in the winters. I sent in a reclaim party to take over the fort after its previous embarrassing death-by-thirst (it being a coastal area, everything was salt water; the well didn't get set up in time as I got distracted, and a fire imp killed the brewer.) It was winter when the party arrived.

Spring came. The many murky pools thawed abruptly- and my fisherdwarf was carrying a shirt across one of them.

I started getting a bunch of death messages one day, after filling a reservoir.

Eventually I located the source.

My reservoir was full of dead babies.

the most disturbing death ive seen is a mother with her baby claiming a craftshop, and then finding out i have no silk...    mother went beserk and tore her child limb from limb...

My dwarven hero went to a mayor, and was told that a vicious beast was terrorizing the town, with 21 kills. Guess what it was.

A cat.

And so, I journey on to kill it. I find it, and attempt to slay it, just to find out that from behind, a archer was actually preparing to attack the cat, and thusly, hits me in the spine, disabling me. Then the cat bites off my head, and then promptly, the archer kills him.

I had a berserk dwarf I locked in a room give birth and then  start attacking her newborn baby.

I think Drakis's is the most disturbing. I had one dwarf accidently lock themself in a tower (apparently ramps don't work sometimes?). He starved to death, or got dehydrated, I don't remember. I also had a flooding accident with a pump, I sent a dwarf to go destroy the pump. A 4/7 tile of water reached him, he dodged out of the way. Straight into the river. I also accidently killed a human by channeling under him, I didn't notice it until I flooded the channel and went to see if it worked or not. It worked but the human was running into a dead and and promptly drowned.

Yikes, the Orcs have a nasty language.  Traditional foreplay would be right out for them; how would they ever "say my name" for one another?  No wonder Ocrs are always so bloodthirsty and violent, they're getting sub-par action.

My entire fortress is about 40 z levels above a river, a big river. Twice now while renovating on the top most level I've caused a cave in. Once with a 1x1 block from 2 levels up. It bunched through 5 or 6 floors of Fortress and one Dungeon Master before exiting the bottom and ending up in the river. I couldn't even find parts of him to bury.

How about "worst near-death"?
I had a dwarf putting up floodgates for my farm room.
He got himself stuck, and I didn't notice till five minutes after.
The only reason I got him out was he was my best miner.
Any other dwarf I would have let starve.

I had to remove the wall I had put up, get a dwarf to put some mechanisms in the floodgate and then pull the lever. Not fun, especially when the wall stayed up for several minutes after giving the order.

The worst part-
It happened twice.

My fisher dwarf was minding his own business when this giant sand scorpion comes along and takes off his leg and stabs him through the chest. He died in his sleep.

I had started a new fortress while reading this thread, everything was going fine. I had found my magma, breached and closed the hole without having anything escape, still all good until i start getting messages half an hour later that my dwarves are dying. Some how a fire man had escaped an loosed all his fury on 4 unnskilled dwarves. 2 died and 1 was cripplled to kill the thing. I thought i could continue through the tragedy... until another fireman appeard and slaughtered the rest of my dwarves.

Nothing had escaped the breach to my magma looking over it there were no way they could have got out. All this happened while reading this cursed thread.

did you have a magma forge/glass furnace operation going?

did you remember to seal the hole with a grate, or a fortification?

I've had more than one death result from forgetting that bridges don't provide support to a construction made next to them, but that a dwarf will try to build something there anyway, and then tumble with the collapsing construction into whatever nasty thing (chasm/lava/river/etc.) is being bridged.  Why I never seem to remember this will happen, I'm not sure.

But really he's just a softie inside. They all are really. When megabeasts come to your fort you never welcome them inside and give them a hug, do you. You heartless bastards...
