I thought you could get hit in both ears because they count as internal organs to the head, is that right?

Simple, really.
The front leg was up near the lung.

Hind leg. IIRC, the arrow also did something to the stomach, but I THINK it was a later hit.

Arrow hits left eye, deflects downward through the mouth (which doesn't show up on the organ lists) and then exits. You flinch so hard you bash your right ear. Technically this is still caused by the bolt so it is called 'pierced.'

I remember hearing on the IRC that these "Magic Arrows" were caused by a programming error that causes weird shit, for example, due to this error, the organs in the upper body are actually wrapped around the ribcage rather than the ribcage around the organs.
I think the same goes for the head, skull, etc.

You're quite wrong.

First off, there's no "rib", "skull', or any skeleton body parts in the game.

Second, the organs are not being treated as if they're outside, the [INTERNAL] tag kinda makes it obvious.

Finally, an over-simplified definition of a creature's body is what causes the odd mechanics on internal damage.

Body parts are (currently) defined very simply. They don't specify an organ's location in the body (well, I guess it can tell you it's in the upper body.) As a result, the game has to randomly determine which organs to hurt.

The new tissue stuff that is coming in the next update could fix the "Magic Arrow", but we'll see.

The flying copper arrow strikes the shopkeeper in the right hand!
It is badly pierced! The shopkeeper's right wrist has been sprained!
The shopkeeper's thumb, right hand has been badly pierced!
The shopkeeper's first finger, right hand has been badly pierced!

At least this one is plausible. Assume this scenario:

The shopkeeper is holding a sword (or something else; doesn't much matter). He has his thumb wrapped around the hilt, and touching his index (or first) finger. The bolt strikes first the center of his index finger, and continues through to the base of the thumb. In the process, the force from the strike causes his hand to be yanked aside, spraining his wrist.

I agree with Urist. Steampunk is like Darth Vader winning Holland's Next Top Model. It would be awesome but not something I'd like in this game.
