dwarfort.exe has encountered a problem and needs to close

I just installed Dwarf Fortress for the first time, and every time I try to create a new world, a window pops up while it is generating saying:

dwarfort.exe has encountered a problem and needs to close.  We are sorry for the inconvenience.

Could someone give me possible problems/solutions for this?

EDIT: I apologize if I posted this on the wrong part of the forums or if I did something completely bone-headed... I just want to play the game.
EDIT #2: Oh, and the version number is v0.28.181.40d.
EDIT #3: Since when have I been an escaped lunatic? :o

Translation, if that was a tad illegible:

Download and unzip DF again (NOT over the old folder), and see if it lets you generate a new world.

hmmm. Now I can't get into DF at all after re-downloading and unzipping the contents to a new place. When I tried to enter DF, an error message came up saying:

FATAL ERROR
main index file missing/corrupted. The file "index" must be in the "data" folder. Make sure DF decompressed into its folders properly.

After reading this, I copied and pasted the index file into the data folder. But when I tried to get into DF after doing this, the game screen just came up jet black and I wasn't able to quit out of it without shutting down. Where did I fuck up so badly?

Are you installing DF onto a flash drive or anything odd like that?

I originally put it on Program Files on the hard drive, then I experimented with several different places and all of them had the same FATAL ERROR message.

EDIT: The Data folder has 3 subfolders, all of which are empty. I think some of the files should have unzipped to there- the problem is I don't know which ones go in which subfolders...

I'll try and upload my own data folder. Replace your original, empty one with mine and see if it works. But don't forget to make backups :)

EDIT: It's a RAR file, is that ok for you? If not, you can either get WinRAR or start complaining and wait till I'm back, so I can zip it :)

The format was fine, but it didn't change anything. I had the same result I had when I copy and pasted the Index file; DF launched but would not start or exit. Its just sitting there with a black screen.

If I were you, I'd back up any data I wanted to see again, immediately, and I'd do it before trying to run scandisk.  Sounds like a hard drive problem.  Those tend to go from "kind of bad" to "oh god the whole thing is unreadable" in the blink of an eye.

But maybe I'm just paranoid after the last two times that happened to me.
