I have a squad of seven marksdwarves called "The Rough Arrows" and a squad of hammerdwarvess lead by Kubuk Deathaxe (realname!) leading Clubs of Creation

Just a question, is a squad name generated by what what weapons its users / leader has?

No, the random number generator is truly random, and squad names are generated for each dwarf when they are generated, either by arriving at your fort or being born. It just so happens that occasionally it is randomly appropriate.

I'll still never forget that lye maker named Murderhammer....

No computer based RNG is truely random, but the RNG used in DF is close enough for all practical purposes.

I do the same thing. I still remember the squad name I got on a volcanic map where I planned to make obsidian knives as my main weapons. 
They were Tharzidun, the Crimson Knives. That was just epic.
I also recall having a squad named "The Inky Truths."

I liked one of my squad's name: "The Lancers of Emancipation". Hooray for freedom-loving dwarves!

That squad was eaten by orcs.

I quite liked the one in my old fortress, "The abbysal floors".  I woulda stationed them on a floor over the bottomless pit if they were in my new one

In my current fort my farmer, should he ever be drafted, will lead The Armoured Gangs. I've made a note to obtain armour before then.

I've just noticed one of my squads is called the Balls of Steel.
The RNG outdoes itself sometimes.
